Service Coordination 

What is Service Coordination?

Medicaid Service Coordination (MSC) assists people with developmental disabilities get access to services, supports and activities they need and desire.

What can service coordination do for you?

  • Service Coordinators help obtain links and referrals to services such as:
  • Advocate
  • Nurture Natural Supports
  • Social Security and Medicaid
  • Respite
  • Day Habilitation
  • Day Treatment
  • Residential Habilitation
  • Pre-vocational Services
  • Supportive Training
  • Provide Family Education & Training
  • Adaptive Equipment
  • Environmental Modifications and Safeguards for the Home
  • Plan of Care Support Services
  • Community Support

Service Coordinators are supportive of the entire family. They maintain regular contact with regards to the person's well-being and the services being provided.

Who is eligible for MSC?

In order to receive MSC, a person must meet all of the following:

  • have a documented diagnosis of a developmental disability before age 22.
  • Be eligible for Medicaid. (children who are under age 18 can apply with a waiver if familial income/resources are too high.)
  • They, or someone authorized to give consent on his/her behalf, must choose to receive MSC.
  • Must not live permanently in another Medicaid funded setting that provides Service Coordination.

The Home & Community Based Services Waiver (HCBS)

HCBS emphasizes a person-centered approach to service planning and delivery. HCBS waiver services are tailored to the individual.

What can HCBS do for you?

The program can provide access to:

  • Respite
  • Day Habilitation
  • Residential Habilitation
  • Environmental Modifications
  • Adaptive Technology
  • Supported Employment
  • Plan of Care Support Services

Who Is Eligible For The HCBS Waiver?

In order to receive the waiver, a person must meet all of the following:

  • Must have a documented diagnosis of a developmental disability before age 22.
  • Must be eligible for Medicaid
  • Must not live permanently in any Medicaid funded setting that provides Service Coordination
  • Cannot be enrolled in any other comprehensive Medicaid long-term care service coordination program
